Terms

What We Need from You

  1. A signed copy of the prescription, with Dentist license number and the State in which the number is registered.
  2. A master model or a non-alginate impression. We do not accept alginate impressions, as they may be inaccurate by the time we receive them.
  3. Please wrap the models/impressions well. Send the case to us by US Mail or by a courier of your choice.
  4. We accept Visa and MasterCard. Contact us with your card information. We will bill the case to your card on the business day before the shipping day.
  5. 3-D printed models are welcomed!

Case Design

If a Dentist-client gives us specific case design instructions and/or drawings, we will implement them. Instructions and drawings are very much encouraged. If no such details are provided, we will use our default style for the appliance.

We want to send you a case that works. Generally, our default style appliances are more comfortable to wear and can be easily adjust as needed at time of delivery.

Warranty

We will warranty all your appliances that we fabricate for 6 months from the date of our invoice against failure or breakage.

The original appliance must accompany a request for a free replacement appliance. If the original appliance is not sent, there will be a full charge for a new appliance. Send a new model or impression with the case to be remade. We will repair, revamp, or remake the appliance as we deem appropriate.

Exclusions:

  1. If the prescription for the appliance has changed; type of appliance, acrylic color, case design or tooth to be banded, the warranty does not apply.
  2. If the integrity of the appliance has been compromised in any way, such as; reduction of the height or thickness of an ortho band, or wires have been ground on or reduced in thickness.
  3. If the appliance has been modified or used in a manner for which it was not originally prescribed or designed, or has reached maximum appliance, opening or expansion.
  4. A passive lingual arch is used in any manner to reposition a tooth or teeth. The Dentist should originally ask for an active lingual arch.
  5. A passive Hawley is modified or added to in order to effect tooth movement. The Dentist should originally let us know what teeth need to be moved.
  6. A fixed or removable expander or other device has been fully activated. If more activation is required, a full fee for a new appliance will be charged.

Case Turn Around Time

Our standard turn around time for a case is 5 working days in the lab. We will call you if we determine that more time is needed for a case.

Mail cases to us by First Class main, or by Priority Mail if you are working in a narrow time frame. We will return all cases to you by First Class mail.
